Palliative Care and Gold Care

Palliative Care 

If you have a serious illness like cancer, congestive heart failure (CHF), COPD, end-stage renal disease, or dementia, you may qualify for an extra layer of support from the Palliative Care team at Devoted Medical. Their specially-trained doctors, nurse practitioners, nurses, social workers, pharmacists, and care coordinators work as a team with YOU at the center. Together, they’ll do all they can to help you live well with serious illness.  

You’ll have visits by video and phone with the Palliative Care team to help you and your caregivers:

• Manage physical and emotional symptoms, like pain, shortness of breath, nausea, constipation, fatigue, depression, and anxiety
• Understand your illness and what to expect in the future
• Plan for your future based on what matters most to you
• Connect to community resources to support you in living well

Palliative care is not hospice care — it helps people in any stage of illness. You can get palliative care along with medical treatments like chemotherapy, dialysis, and hospital-level care. Studies show that people who get palliative care, together  with medical care for their serious illness, feel better and live longer.

Gold Care

Sometimes, getting the right care means more than staying on track with checkups and medications. Maybe you have several health issues. Or mental health challenges, like depression or anxiety. Or loneliness. Or issues with housing. Or a mix of all of the above and then some. Gold Care is here for you. 

With Gold Care, you have a team committed to getting all your needs met, whether they’re medical, emotional, or social. You’ll have a community health worker, nurse, nurse practitioner, social work case manager, doctor, and outreach coordinator working together to help you live a healthier, happier life.

The Gold Care team helps:

• Give you one place to turn for your questions and concerns — they’re looking at the big picture and the tiny details
• Simplify your medical care by coordinating with your specialists and other providers
• Find you mental health support, either in-person or virtually (by video or phone)
• Connect you with community resources to address loneliness, housing, employment, and more
