Welcome to your dental and eyewear benefit
With certain Devoted Health plans, you get a card that’s loaded with a yearly dental and eyewear allowance. To see how much you get each year to spend on dental care and eyewear, check your plan documents.
The information on this page is for 2025 Devoted Health plans (see 2024 details).
Note: Not all Devoted Health plans have a Dental & Eyewear Card. If the back of your Member ID card says “Dental & Eyewear Card” — then your plan does! If not, check your plan documents or see our dental benefit overview to learn more about your plan’s dental and eyewear benefits.
What can I use my card for?
- Routine dental cleanings, exams, and X-rays
- Comprehensive dental care — like fillings, crowns, or dentures
- Eyewear (glasses, contact lenses, or both)
You can use your card at any dentist or eyewear provider you choose. You can’t use your card for dental implants, dental prosthetics, orthodontics (like braces or Invisalign®), or cosmetic dentistry (like whitening treatments or veneers).
Note that your plan also covers a yearly eye exam, separate from the allowance on this card. Check your plan documents for details.
Do I need to use the full amount all at once?
No — you can make as many separate purchases as you want for covered services, as long as there’s money left on your card. To check how much you have left, log in to your card’s portal.
Can I use my card at any dentist or eyewear provider?
Yes — your card should work at any dentist or eyewear provider that takes MasterCard.* Just say you’re “self-pay” and use your card. Otherwise, you’ll pay upfront and we’ll pay you back (as long as your purchase qualifies).
TIP: If your card doesn’t work at a location where it should, no problem. Just pay out of pocket and we’ll pay you back. It’s quick and easy!
How can I get paid back for purchases I make without the card?
It's quick and easy. Log in to your card’s portal and tap the Request Payment button under Home to get started. If you run into issues, text us at 866-85 or give us a call at 1-800-338-6833 (TTY 711).
Keep in mind that when you buy something without your card, it won’t show up on your card balance until after we’ve paid you back. So if you see a higher balance on your card than expected, remember that you’ve already spent some of that money!
How can I check my card balance?
Log in to your card’s portal, text us at 866-85, or call us at 1-800-338-6833 (TTY 711).
Do I have to see in-network providers?
Nope, not for covered purchases you pay for with your card. You can see whatever provider you choose. For your yearly eye exam and other medical care, you may need to stay in-network or there may be a lower cost at in-network providers, depending on your plan. Check your plan documents for details.
What if I lose my card?
Call us right away at 1-800-338-6833 (TTY 711). We’ll send you a new card and make sure that no one can use the old one.
If I want to spend more than I have left, can I pay the rest in cash?
Yes. Dentists and eyewear providers that take your card should be able to use your card balance, then let you pay the difference by cash or credit card.
For dentists and eyewear providers that don’t take your card, you’ll pay the entire amount and we’ll pay you back for the balance that was on your card.
TIP: Let’s say you have $300 left on your card and you’re paying a $400 dental bill. Be sure to tell them to put only $300 on your card. If they try to charge the whole amount, your card will get denied.
*The card will only work at providers who accept MasterCard and have a Merchant Category Code (MCC) of 8021-Dental, 8071-Dental, 5047-Dental, 8042-Vision, or 8043-Vision. To use the benefit at other providers, pay upfront and submit for reimbursement.
