Get paid back for covered care
If you pay for something that your plan covers — like healthcare services or Wellness Bucks purchases — you can ask us to pay you back for the covered amount. Here's how.
For most requests
For purchases related to your white and silver dental Mastercard, Food & Home Card, or prescription drugs, see the sections below.
For everything else, ask online or by mail to get paid back (reimbursed).
Online
This is the quickest, easiest way to get paid back:
- Complete our reimbursement form.
- Attach your receipt. For healthcare and dental purchases, be sure to also attach a detailed bill or invoice.
- Keep an eye on your mailbox. As long as your form has everything we need, you'll get your check within 3 weeks.
By Mail
Print and return a completed reimbursement form.
Don’t have a printer? Text us at 866-85 or call us at 1-800-338-6833 (TTY 711). We’ll mail you a copy.
The form is an easy way to make sure you’re sending us all the right information. But if you prefer not to use it, you can send us the same information on a separate sheet of paper instead.
We usually process requests within 30 days, but it may take up to 60 days.
For your white and silver dental Mastercard
Some plans have a pre-paid Mastercard to pay for dental benefits. If you weren’t able to use your card and paid out of pocket for a dental expense, log into your card’s account to ask to get paid back.
For your Food & Home Card
Some plans have a Food & Home Card. If you weren’t able to use your card and paid out of pocket for a Food & Home Card expense, fill out the online CVS/OTC Health Solutions reimbursement request form. Or call CVS at 1-855-904-3663 (TTY 711) to get a paper form.
For prescription drugs
Print and return a completed Prescription Drug Reimbursement Form.
