Joining our network

What to expect after completing a participation request

Participation requests are reviewed individually and compared to our current network need, so please allow 30-60 days for our evaluation. If it is determined that Devoted Health has a network need, a team member will contact you to discuss contracting opportunities.

If you do not hear from us, you do not need to contact Devoted Health. This simply means we are not contracting with additional providers of your specialty at the time of submission. We will retain your information and may contact you should our needs change.

Credentialing information

Completing the participation form does not constitute a credentialing application. Credentialing is handled as a separate process after contracting and the team will reach out for instructions.
