Provider Data & Credentialing
Updating Provider Data
Our online Provider Directory is updated daily. Provider updates, adds, changes, and terminations should be sent monthly (at a minimum) to provider-updates@devoted.com or through SFTP using our roster templates below.
Required Fields for Provider or Facility Rosters
- NPI
- TIN
- Provider's First & Last Name or Facility Name
- Office or Facility Name
- Office or Facility Address
- Office or Facility Phone
- Office or Facility Fax #
- Provider's Specialty or Facility Type & Services
- Request to exclude from Directory (Y/N) (Provider Roster Only)
- Panel Status - Open, Closed or Existing Patients only (Provider Roster Only)
- CAQH (if applicable for Provider Roster Only)
Failure to provide any of the above information will result in delays in roster processing.
Credentialing
Practitioner Credentialing
Practitioner credentialing starts once a contract is executed and the practitioner is loaded into our system via roster updates. We utilize CAQH applications to complete credentialing. To avoid delays in the credentialing process, please ensure Devoted Health is authorized to access your CAQH profile and all information in CAQH is up-to-date.

Devoted’s Credentialing Verification Organization (CVO) or the Devoted Credentialing team will reach out to the credentialing contact on file if any information is missing.
Once credentialing is approved, an email notification will be sent to your group’s credentialing contact.
Effective Dates: A practitioner’s effective date(s) can be found in the Devoted Provider Portal (log in via Availity):
- You may receive an email from us when your credentialing process is complete, if you do not receive this email, you can check your credentialing status and effective date(s) on Devoted Provider Portal via Availity. Learn about our provider portal.
- If you do not have access to Availity, please contact Provider Services to obtain effective dates by calling 1-877-762-3515. The Provider Service team is available Monday through Friday, 8am to 5pm, local time.
Facility & Ancillary Provider Credentialing
The credentialing application can be used to send us facility credentialing information once your group is contracted. Instructions are included in the application. You can find the application in Availity under Devoted Health's payer space.

Ongoing engagement
- As providers are approved, we’ll send approvals to your group’s credentialing contact.
- We monitor all credentialed providers and recredential every 3 years
