Transition your care (HMO — Advocate Health)
An important part of joining a new plan is making sure you’re all set to get the care you need from Day 1.
Note: This page is only for HMO plan members in Illinois who have a primary care provider (PCP) in the Advocate Health system. If that doesn’t apply to you, see one of these pages:
Your Illinois HMO plan only covers in-network care (except for emergency care), so you’ll need to make sure all your doctors are in our network. And your Advocate Health PCP works closely with certain trusted specialists and medical suppliers, so you’ll also need to make sure your care is the Advocate Health system.
But we know it can take time to get set up with new providers. So, if you’re getting active care, we give you up to 90 days to make the switch (and to set up any approvals or prior authorizations you may need).
What counts as active care?
Active care includes things like:
- Medical equipment you’re renting, like a CPAP machine, oxygen, or power wheelchair
- Ongoing cancer treatment, like chemotherapy or radiation
- An appointment to follow up from a recent surgery
- A surgery that’s already been scheduled or approved with your previous plan
Active care does NOT include:
- A first-time appointment with a new doctor
- A routine screening, like a yearly mammogram
- A yearly or 6-month follow-up visit with a doctor
What if I use medical supplies or rent medical equipment?
For a power wheelchair or scooter that you rent
Orbit Medical and Numotion are our in-network suppliers. If you’re renting a power wheelchair or scooter from a different company, ask your doctor to send a new order to Orbit or Numotion:

For diabetes supplies
It depends on the specific brand that you use. If you use a brand that’s not listed here, give us a call at 1-800-DEVOTED and we’ll help you figure out if it’s covered.
Diabetes supplies (available at in-network pharmacies)
- OneTouch glucometers and supplies
- Freestyle Libre continuous glucose monitor
- Omnipod Kit, Dash, or V-Go insulin pump and supplies
- Dexcom continuous glucose monitor (available in pharmacies starting January 1, 2025)
You can get these at any in-network pharmacy. Just have your doctor send an order to the pharmacy.
Diabetes supplies (available only through a vendor)
- Dexcom continuous glucose monitor
- Tandem T Slim insulin pump
- Medtronic Minimed insulin pump
Ask your doctor to:
- Submit a prior authorization through Advocate.
- Send an order to Byram at 1-800-775-4372 (fax: 866-478-9348)
For any other medical equipment or supplies
Advocate at Home is your one-stop shop. If you’re currently using a different supplier, ask your provider to send a new order to Advocate at Home (by phone at 630-368-5900, or by fax at 630-368-5988).
Tip: If you own your medical equipment and it doesn’t use any supplies, you don’t need to take any action. If you’ve been renting your equipment for more than 13 months, you may actually own it and will not need to return it — check with your supplier to be sure. The one exception is oxygen concentrators, which always need to be returned if you’re changing suppliers.
What if I have upcoming office visits, surgeries, procedures, or treatments?
For behavioral health or mental healthcare
Call our partner Magellan at 1-800-776-8684. They can help you check if your behavioral health provider is in-network — and help you find a new provider if needed.
For any other care
The first step is to check with your Advocate Health PCP. They can help you make sure your specialists are in-network with both Devoted Health and Advocate Health and put in any needed referrals — or connect you with a new in-network Advocate specialist if needed.
If you’re getting active care, you can keep seeing your out-of-network or non-Advocate providers for up to 90 days. Not sure if your care qualifies as active care? Give us a call at 1-800-DEVOTED or ask your provider about submitting a prior authorization request — that way we can make sure there are no surprises.
Note: Some out-of-network providers may refuse to see you once your new plan starts — even if you qualify as getting active care during the 90-day window. If this is the case and you need help finding an in-network provider right away, give us a call and we’re happy to help.
Why can I only see Advocate Health providers?
Advocate Health PCPs work closely with certain trusted specialists in the Advocate Health system to manage their patients’ care. If you want to keep seeing a specialist who’s in the Devoted Health network, but your Advocate PCP will not refer you to them, you have the option to switch to a different PCP so you can keep seeing that specialist.
